Your personal data is processed in the medical documentation, in full compliance with the valid legal regulations, especially in compliance with Act No. 372/2011 Coll., about the services and conditions of their provision (Health Care Services Act) and its implemented regulations. Its security and protection is ensured in compliance with these regulations and in compliance with the General Regulation for Protection of Personal Data 2016/679 (GDPR).
In relation to your personal data, based on the above stated, you have the following rights:

• The right to access your personal data stored at our worksite;
• The right to correct your personal data stored at our worksite;
• The right to restrict your personal data stored at our worksite;
• The right to object to the processing of your personal data stored at our worksite;
• The right to file a complaint at the Office for Personal Data Protection.

Providing your personal data is a legal request and as a Patient, you have an obligation to provide your information and the health care employee has the right to request that you provide your personal information, given that this worksite does not request additional personal data, above and beyond the fulfilment of the legal obligations of the health care employee, without your explicit request. We store such personal data in a secure place and for the time necessary, as defined by the legal regulations. This personal data is then deleted properly and completely.

Failure to provide our worksite with your personal data means that it will not be possible to provide you with medical services, which could lead to the worsening of your health condition or directly threaten your life.
